Declaratie privind eligibilitatea; Declaratie privind neîncadrarea în situatiile prevazute la art. 180 si 181 din OUG nr. 34/2006; Fisa de informatii generale; Cazier judiciar al persoanelor fizice, aflate la conducerea operatorului economic; Certificat de atestare fiscala eliberat de organul de administrare fiscala al unitatii administrativ teritoriale de pe raza careia societatea are sediul social, privind plata obligatiilor la bugetul general consolidat, care sa ateste ca societatea ofertanta nu se înregistreaza cu debite la bugetul consolidat, la data de 31.07.2008, în original sau copie legalizata, valabil la data deschiderii ofertelor; Certificat fiscal eliberat de Compartimentul Impozite si Taxe Locale al unitatii administrativ-teritoriale de pe raza careia societatea are sediul social, care sa ateste ca societatea ofertanta nu se înregistreaza cu debite la bugetul local, la data de 31.07.2008, în original sau copie legalizata, valabil la data deschiderii ofertelor (Cerinta minima), certificat constatator emis de Oficiul Registrului Comertului de pe lânga Camera de Comert si Industrie nationala sau teritoriala, cu cel mult 30 de zile înainte de depunerea documentelor de calificare, în original sau copie legalizata (Cerinta minima).

a) Lista a lucrarilor executate în ultimii 5 ani, însotita de certificari de buna executie pentru cele mai importante lucrari si care vor contine valoarea, perioada si locul executiei lucrarilor, modul de îndeplinire a obligatiilor, beneficiari, indiferent daca acestia sunt autoritati contractante sau clienti privati (cerinta minima);
Ofertantii vor face dovada finalizarii (adica receptie la terminarea lucrarilor sau receptie finala) in ultimii 5 ani, a unui contract de executie a unei sali de sport sau a unei constructii din confectii metalice de dimensiuni asemanatoare, cu o valoare mai mare sau egala cu 1.766.200 lei/1 lucrare (echivalentul a 500.000 Euro, 1Euro = 3,5324 RON, la 21.08.2008), al carui obiect a fost executia unor lucrari similare. (cerinta minima)
b) Personalul/organismul tehnic, de specialitate, de care dispune sau al carui angajament de participare a fost obtinut de catre candidat/ofertant, în special pentru asigurarea controlului calitatii. (cerinta minima).
