declaratie pe propria raspundere privind eligibilitatea; declaratie privind neîncadrarea in art.181 din OUG34/2006; declaratie privind calitatea de participant la procedura; certificate privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor; cazier fiscal; certificat constatator; certificat de înregistrare sau documente care dovedesc o forma de înregistrare/atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile din tara în care este rezident.

bilantul contabil întocmit la 31.12.2007, în copie, vizat si înregistrat de organele competente; declaratie privind cifra de afaceri pe ultimii trei ani (2005-2007). Nu vor fi acceptati ofertantii din ale caror documente rezulta ca cifra de afaceri medie pentru ultimii trei ani este mai mica decât nivelul specificat minim necesar.

declaratia privind lista lucrarilor executate în ultimii 5 ani; lista trebuie sa contina si inf. privind executarea a cel putin unui contr. similar în val. min. spec. Este oblig. ca în per. mentionata, ofert. sa faca dovada îndepl. si final. unor contr. de lucr. ce au avut ca obiect executarea de lucr. similare celor ce fac ob. proced., resp. prezentarea pvrtl si/sau a pvrf, încheiate ptr. lucr. declarate în lista; se va prezenta obligatoriu pvrtl si/sau pvrf , încheiat pentru lucr. ce fac obiectul contr. similar în val. min. specificata, declarat în lista; declaratie privind partea /partile din contract care sunt îndeplinite de subcontractanti si specializarea acestora, însotita si de acordurile de subcontractare; declaratie privind utilajele, inst., echip. tehnice de care dispune op. ec. pt. îndeplinirea corespunzatoare a contr. de lucrari împreuna cu lista cuprinzând dotarile specifice cu utilaje, echip. tehnologice etc. avute în patrimoniu, sau acordurile de subcontractare pentru dotarile necesare; declaratie privind efectivele medii anuale ale personalului angajat si al cadrelor de conducere, împreuna cu CV-urile personalului de conducere, precum si ale personalului responsabil pentru îndeplinirea contractului de achizitie publica; pentru resp. tehn. cu executia se va prezenta, în copie legalizata, doc. de atestare, în termen de valab.; lista cu pers. de conducere, pers. tehnic de spec., precum si pers. resp. ptr. îndepl. contr.; certificat de conformitate cu standardul ISO 9001/2001 privind sistemul de management al calitatii implementat în cadrul firmei, emis de un organism acreditat; dosarul sistemului de management de asigurare a calitatii lucrarilor si materialelor utilizate si livrate, propus pentru realizarea contractului; declaratie privind asig. stand. de prot. a mediului si faptul ca la elab. ofertei, s-a tinut cont de oblig. ref. la cond. privind mas. de prot. a mediului pe care op. ec. le poate aplica în timpul îndepl. contr. de lucrari; declaratie prin care ofertantul sa dovedeasca faptul ca la elab. ofertei a tinut cont de oblig. ref. la cond. de munca si prot. muncii stipulate prin legislatia muncii în România.
