Note: "District Secretary - Polonnaruwa" for a sum of Rs. 250/ = indicating the paying office as "Sub- Post Office, New Town, Polonnaruwa" must be sent along with the application as non-refundable registration fee.

Applications are invited from suppliers, agencies, distributors, business organizations, manufacturers who wish to register themselves with the Polonnaruwa District Secretariat for the supply of under mentioned supplies and services for the year 2009.
Suppliers who seek registration under items i, xiv, xv must submit copies of the registration certificates which shows their registered respective limits with the Institute for Construction and Development (ICTAD).
Suppliers who seek registration for more than one item should produce separate application for each item for which registration is sought. Money order written in favour of "District Secretary - Polonnaruwa" for a sum of Rs. 250/ = indicating the paying office as "Sub- Post Office, New Town, Polonnaruwa" must be sent along with the application as non-refundable registration fee.
Application forms will not be issued and applicants are advised to make applications themselves providing following details.
Normally, quotations for supplies and services will be called from the list of registered suppliers, but, quotations will be called from suppliers outside the registered list whenever District Secretary thinks necessary. District Secretary reserves the right to purchase.
Suppliers who fail to produce quotations or execute the orders placed within them to the satisfaction of the District Secretary will be deleted from the list without any prior notice. District Secretary reserves the right to accept or reject any application without any reasoning.
Applications can be sent through registered post or by hand to "District Secretary, District Secretariat, Polonnaruwa" on or before "30th December 2008" and top left hand corner of the envelope should be clearly marked "Registration of Suppliers for the year 2009".
