Note: 1) For metal in every size and river sand - Rs. 1000.00+ 15% VAT, 2) For tar and emulsion - Rs. 3000.00 + 15% VAT, 3) For tyre/tube - Rs. 1000.00 + 15% VAT, 4) For supplying of asphalt concrete - Rs. 1000.00 + 15% VAT, 5) For each subject in all other service supplying - Rs. 500.00 + 15% VAT

Applications are called for the registration of Suppliers and Service Providers for this Authority for the year 2009.
2. Conditions of Registrations - Applicants who wish to be registered as suppliers could either collect the relevant application for registration by calling over at the Supplies Division on submitting the receipt obtained for payment of non-refundable application fee to the Cashier of this authority by cash as mentioned below for each heading of supplies from 9.00 a.m. to 3.00 p m on any working day of the week or coiled obtain through mail by sending a bank draft for the application fee mentioned below together with a copy of the business registration certificate to the following address.- 1) For metal in every size and river sand - Rs. 1000.00+ 15% VAT, 2) For tar and emulsion - Rs. 3000.00 + 15% VAT, 3) For tyre/tube - Rs. 1000.00 + 15% VAT, 4) For supplying of asphalt concrete - Rs. 1000.00 + 15% VAT, 5) For each subject in all other service supplying - Rs. 500.00 + 15% VAT
3) Also separate applications should be submitted for each supply and service item above.
Applications for Registration of Suppliers will be issued up to 2.00 p.m. on 10th December 2008 from 10th November 2008. Request make beyond the said time will be rejected. Duly filled applications could either be deposited at the application box kept at the 2nd floor of the Authority before the closing date or be sent by registered post to the following address "Registration for Supply purposes 2009"should clearly be marked at the top left-hand comer of the envelope.
Business Registration Certificate and other certificates requested by the authority in order to confirm the identification of the Authorized Officer should be enclosed with the application.
The authority reserves the right to accept or reject the applications.
