Applications are invited from Multi Purpose Co-operative Societies, Cooperative Wholesale Establishment, Other registered Co-operative Societies, Recognized Commercial Enterprises and Organizations who wish to register themselves with Supplies Division, Ashraff Memorial Hospital for the supply of following items for the year 2009.
Application forms for registration can be obtained from the Office of the Finance Division, Ashraff Memorial Hospital, Kalmunai on working days from 9.30 a.m. to 2.30 p.m. until 05.12.2008 on production of receipts obtained after payment of non refundable fee of Rs. 300/- in respect of each item.
Duly perfected forms stated above in duplicate marked "Original & Duplicate" should be sent _to the Medical Superintendent, Ashraff Memorial Hospital, Kalmunai under; registered cover or can be deposited in the Tender Box kept for purpose at the Office of Finance Division on or before 10.00 a.m. on 06.12.2008.
"Registration of Supplies for 2009" should be printed on the top left-hand corner of the envelope enclosing application.
