This Sources Sought Notice is not a request for formal proposal. This Notice is to identify Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB)qualified as such under PL 109-461 who are able to provide commercial ambulette or non-emergency medical coach services 24 hours, 7 days per week to the Phoenix Veterans Affairs Health Care System (PVAHCS)(650 E Indian School Road, Phoenix, AZ 85012-1892) and two (2) Community Based Outpatient Clinics (SE Extension Clinic and NW Extension Clinic both located in the Phoenix metropolitan area). Vendor shall furnish Wheelchair Type vehicles (Medi-Coachs). Services shall include vendor having an established facility(ies) in operation for at least one (1) year where ambulette units are housed and dispatched from a location within a thirty (30) mile radius of the PVAHCS' installations. Intended contract to be awarded on/about February 15, 2009. All responses shall include identification/certification of company size status and any pertinent information pertaining to this sources sought. This synopsis is for information and planning purposes and is not to be construed as a commitment by the Government.
