This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is N40442-09-T-4004, and it is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. NAICS 811310 applies. The Small Business Competitiveness Demonstration Program is not applicable. Military Sealift Fleet Support Command (MSFSC), Norfolk, VA intends to award a firm fixed price purchase for work to be performed onboard the USNS COMFORT(T-AH 20) for the following: 1. Statement of Work: A. Provide the services of a qualified and licensed deck repair company to perform the following work in MTF spaces: NOTE: NO MORE THAN 2 AREAS SHALL BE TAKEN OFF LINE AT ANY ONE TIME. The ship must maintain her �Ready 5� (5 day activation window) while repairs are in progress. 1. Remove existing VCT and provide and install PRC with vinyl cove base on the following spaces: � Barber Shop on 2nd deck. � Ships Laundry (3-38-0) � Ships Laundry (3-39-0) � Ships Laundry (3-41-0) 2. Accomplish the following work in MTF GALLEY: � Prepare and apply electrostatic paint on the entire overhead. � Provide and install new supply fan ducting diffusers with filters. Provide as set of spare filers. � Provide and install two (2) overhead access hinged panels � Prepare and paint the entire bulkheads and trims as per original. � Repair and reseal 25 sq.ft damaged terrazo. 3. Reseal terrazo in toilet, sink and shower area and caulk cove at bulkheads in Male Crew Toilet (2-61-2): 4. Reseal terrazo in toilet, sink and shower area and caulk cove at bulkheads in Male Crew Toilets (3-61-2) and (3-61-2). 5. Accomplish the following repairs in Female Crew Toilet/Shower (3-61-1): � Reseal terrazo in toilet area and caulk cove at bulkheads � Replace 25 sq.ft terrazo in sink area and reseal the remainder of the area and recaulk cove at bulkheads. � Replace terrazo inside the showers and reseal terrazo on the remainder of the space and recaulk cove at bulkhead. 6. Replace terrazo inside showers and reseal terrazo in the remainder of the space and recaulk cove at bulkhead of Female Crew Toilet/Shower (3&4-61-1) Provide the port engineer with a condition found report. Any additional work shall be covered under a change order. B. (A) The taxpayer has received a statutory notice of deficiency, under I.R.C. �6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights. (B) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. �6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights. (C) The taxpayer has entered into an installment agreement pursuant to I.R.C. �6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment. (D) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. �362 (the Bankruptcy Code). (3) If paragraph (k)(1) or (k)(2) of this clause applies� (i) If the offeror does not certify to the conditions in paragraph (k)(1) or (k)(2) and the Contracting Officer did not attach a Service Contract Act wage determination to the solicitation, the offeror shall notify the Contracting Officer as soon as possible; and (ii) The Contracting Officer may not make an award to the offeror if the offeror fails to execute the certification in paragraph (k)(1) or (k)(2) of this clause or to contact the Contracting Officer as required in paragraph (k)(3)(i) of this clause. (l) Taxpayer Identification Number (TIN) (26 U.S.C. 6109, 31 U.S.C. 7701). (Not applicable if the offeror is required to provide this information to a central contractor registration database to be eligible for award.) (1) All offerors must submit the information required in paragraphs (l)(3) through (l)(5) of this provision to comply with debt collection requirements of 31 U.S.C. 7701(c) and 3325(d), reporting requirements of 26 U.S.C. 6041, 6041A, and 6050M, and implementing regulations issued by the Internal Revenue Service (IRS). (2) The TIN may be used by the Government to collect and report on any delinquent amounts arising out of the offeror�s relationship with the Government (31 U.S.C. 7701(c)(3)). If the resulting contract is subject to the payment reporting requirements described in FAR 4.904, the TIN provided hereunder may be matched with IRS records to verify the accuracy of the offeror�s TIN.
